Forms

Many times you need to take care of business when you are not on campus. Because we need documentation from you in order to initiate changes to your records, forms must be used. The following forms can be downloaded for you to fill out and submit in person after meeting with your Academic Advisor, or fax in at your convenience.

All forms can be faxed to the attention of the Registrar at 310.314.6053. Please note deadlines, required signatures, fees, and processing times indicated on all forms.

FERPA WaiverSpecify who AiCALA can release your information to
Address/Telephone/Email ChangeUpdate your contact information with AiCALA
OLS Password ResetRequest or reset your password for you OLS account
Change of MajorChange your program of study to a different major
Transcript RequestRequest official transcripts
Full Time PlusRequest permission to take more than 18 units/credits in a quarter
Graduation ApplicationPending grads complete this form when registered for their final quarter
Withdrawal RequestFor those wishing to withdraw from AiCALA
Course WithdrawalTo drop a course after the drop/add period
